ABSTRACT

La Dermatitis Digital (DD) es una afección podal de distribución mundial. Al igual que otras enfermedades que afectan al pie bovino induce evidentes cojeras, con las consecuentes pérdidas productivas y económicas. Comúnmente se la describe en el ganado lechero y raramente en ganado de carne. En este trabajo se presentan dos brotes de DD en novillos de feedlot del Noroeste Argentino. Se destacan aspectos clínicos, epidemiológicos, patológicos y de diagnóstico. La incidencia fue de 5 y 35 %. Las lesiones se identificaron típicamente en el área palmar de los miembros posteriores e histopatológicamente se observó una dermatitis necrotizante con infiltrado neutrofílico. Mediante la tinción Warthin-Starry se identificaron espiroquetas intralesionales en el 50% (4/8) de las muestras colectadas.


Digital Dermatitis (DD) is a podal condition of worldwide distribution. Like other diseases that affect the bovine foot, it induces obvious lameness, with consequent productive and economic losses. It is commonly described in dairy cattle and descriptions in beef cattle are scarce. In this paper, two DD outbreaks are described in steers from feed-lot in the Argentine Northwest. Emphasis is given to more relevant aspects of clinical, epidemiological, pathological and diagnostic findings. The incidence was 5 and 35%. Lesions are typically identified in the palmar area of the hind limbs, and a necrotizing dermatitis with neutrophil infiltrate was observed histopathologically. By Warthin-Starry staining was identified an intralesional spirochetes in 50% (4/8) of collected samples.

ABSTRACT

Introduction: Non-communicable chronic diseases (NCCD) increase the presence of risk factors (RF) of cardiovascular disease (CVD) in young people. Objective: To analyze the relation among the nutritional status (NS), physical activity (PA) and the prevalence of cardiovascular RF in students from Santo Tomas University in Temuco enrolled in year 2010. Subjects and methods: A cross-sectional descriptive study was carried out; 177 out of 348 students of both genders were chosen as the sample, 32.2% males and 68.8 % females. The age ranged between 18 and 25 years old. Anthropometric and blood pressure measurements, blood samples, PA surveys and obesity backgrounds were taken. The study outcomes helped to estimate the NS, the practice of PA, the prevalence of the different RF. Outcomes: The study showed that the 34.5% of the students present malnutrition by excess, 70.6% are sedentary, and a 9.6% is at risk of CVD by presence of metabolic syndrome (MS). The risk by abdominal obesity reached 53.1%. Parameters which showed significant differences were HDL cholesterol lowered in active students (p= 0.027). When the PA together with BMI variables, triglycerides, LDL cholesterol, glycaemia and blood pressure were analyzed, no significant differences were observed. Conclusions: A high prevalence of malnutrition by excess, for being sedentary and RF of CVD was present with a predominance in female students.


Introducción: Las enfermedades crónicas no transmisibles (ECNT) incrementan la presencia de factores de riesgo (FR) de enfermedad cardiovascular (ECV) en jóvenes. Objetivo: Analizar la relación entre estado nutricional (EN), actividad física (AF) y prevalencia de FR cardiovascular en estudiantes de la Universidad Santo Tomás sede Temuco, ingreso 2010. Sujetos y métodos: Se realizó un estudio descriptivo de corte trasversal; de un total de 348 estudiantes, se obtuvo una muestra de 177 sujetos, de los cuales 32,2% correspondió a hombres y 68,8% a mujeres, la edad fluctuó entre 18 y 25 años. Se realizaron mediciones antropométricas, de presión arterial, muestras sanguíneas, encuestas de AF y antecedentes de morbilidad. Los resultados permitieron estimar el EN, la práctica de AF y la prevalencia de los distintos FR. Resultados: El estudio reveló que 34,5% de los estudiantes presentaban tiene malnutrición por exceso, 70,6% eran sedentarios y 9,6% tenían riesgo de enfermedad cardiovascular por presencia de Síndrome Metabólico (SM); mientras, el riesgo por obesidad abdominal fue de 53,1%. Los parámetros que evidenciaron diferencias significativas fueron el colesterol HDL que se mostró disminuido en los estudiantes activos (p= 0,016) y el colesterol total, que se observó levemente aumentado en los sedentarios (p= 0,027). Al analizar la AF con las variables IMC, triglicéridos, colesterol LDL, glicemia y presión arterial, no se observaron diferencias significativas. Conclusiones: Se observó alta prevalencia de malnutrición por exceso, sedentarismo y factor de riesgo de ECV con predominio en las mujeres.

ABSTRACT

We report a 59-year-old man with a history of hypertension, recurrent renal stones and a severe hypercalcemia of 14.9 mg/dl with a serum phosphorus of 2.4 mg/dl and a serum albumin of 3.6 g/dl. Physical examination showed a 4 cm left cervical nodule, consistent with the diagnosis of thyroid nodule. Parathyroid hormone (PTH) levels were 844 pg/mL (normal 15-65 pg/ml) and a cervical ultrasound examination disclosed a solid nodule in the lower left lobe of 40 x 30 x 25 mm, adjacent to the thyroid parenchyma. Abdominal ultrasound revealed bilateral renal stones. Parathyroid scintigraphy showed a high uptake of the left lower parathyroid mass and a bone densitometry showed bone density t scores of -1.2 in the spine, -2.0 in the right femoral neck and -3.5 in the distal radius. A review of his medical record revealed the presence of hypercalcemia for at least 4 years. He was admitted for hydration and administration of 4 mg zoledronic acid iv. At 24 hours, serum calcium dropped to 11.0 mg/dl, and a left thyroid lobectomy was performed including the lower left parathyroid gland. The pathology report showed a 22.6 g parathyroid adenoma. Intraoperatory PTH descended > 50%, consistent with successful parathyroidectomy. At 7 days after surgery serum calcium was 8.8 mg/dl, phosphorus 2.1 mg/dl, alkaline phosphatase 166 U/L, albumin 3.9 g/dL, PTH 230 pg/ml and 25-OH vitamin D 12.4 ng/ml. This finding was interpreted as secondary hyperparathyroidism due to vitamin D deficiency and “hungry bone”, being less likely the presence of residual or metastatic parathyroid tissue. A cholecalciferol load was administered, with significant descent of PTH.

ABSTRACT

Although prostacyclin (PGI2) production in the umbilical artery is known to be reduced in prognancy-induced hypertension (PIH), little information is available about its production in maternal vascular tissues. We measured 6-keto-prostaglandin F 1* generation in the umbiblical and comentum arteries of 24 Andean women divided into three groups: 1) 8 normal pregnant women, 2) 8 cases with clinical evidence of severe PIH, and 3) 8 normotensive non-pregnant women.The normal pregnant group (232 ñ 172 pg mg-1 2h-1) and the non-pregnant control group (237 ñ 146 pg mg-1 2h-1 0 showed similar PGI2 production in the omentum arteries, whereas the PIH group showed lower PGI2 generation (P<0.05) than the normal patients both in the umbilical (697 ñ 377 vs 1528 ñ 291 pg mg-1 2h-1) and omentum (132 ñ 73 vs 232 ñ 172 pg mg-1 2h-1) arteries. PGI2 production was 6.8 times lower in the omentum arteries than in the umbilical arteries. The data confirm and extend the view of the occurrence of reduced PGI2 production in the maternal-fetal vascular tissues of women with severe PIH.

ABSTRACT

La angioplastia transluminal percutanea (ATP), permite la dilatacion de una estenosis o la desobstruccion vascular por medio del cateterismo. Tecnica introducida por Dotter en 1964, en los ultimos anos ha experimentado gran difusion mundial a raiz del uso del cateter de Gruntzig. Practicamente en todos los territorios vasculares se han efectuado angioplastias, siendo sus indicaciones y resultados a largo plazo, todavia no totalmente definidos. Sin embargo el metodo ha sido aceptado y sus buenos resultados comprobados, cuando la cirugia no permite realizar procedimientos de revascularizacion o estos involucran un importante riesgo para el enfermo.Nuestro interes es que el primer caso exitoso de ATP realizado en nuestro pais, sea el comienzo de un trabajo a largo plazo para valorar las posibilidades de dicho metodo terapeutico
